Job Description:
The Program Coordinator provides administrative supervision to staff and reviews case recording to ensure that records are in compliance with funder and internal standards. The Program Coordinator also ensures that intakes, assessments and treatment plans are completed on time, are consistent in terms of client presentation and goals of service, and that staff are attentive to the goals outlined in treatment plans.

Case Manager - Association for Rehabilitative Case Management and Housing, Inc.

Case Manager, Licensed Residential Program, Queens

Responsibilities:
· Conducts home visits on a regular basis for assigned case load.
· Obtains psychosocial histories and conducts functional skills assessments.
· Establishes and maintains liaison with primary provider of psychiatric care.
· Assures access to psychiatric, substance abuse and medical services, and entitlement benefits.
· Monitors client medication, including adherence and side-effects.
· Develops an individualized rehabilitation plan (Service/Support Plan) in accordance with client needs, strengths, and goals and in consultation with the treatment team and others of the client’s choice.
· Delivers community rehabilitation services according to individual need, including: Assertiveness/Self Advocacy Training, Community Integration Services/Resource Development, Daily Living Skills Training, Health Services,
· Medication Management Training, Parenting Training, Rehabilitation Counseling, Skill Development Services, Socialization, Substance Abuse Services, and Symptom Management.
· Documents client response to service, progress and regressions, and regularly updates the Service/Support Plan.
· Trains clients in safety procedures and conducts fire drills and self-preservation tests.
· Prepares residents for transition to independent living and makes necessary referrals.
· Facilitates crisis intervention in the event of an emergency.
· Staffs Emergency Pager on a rotating basis.
· Maintains client charts and submits necessary notes and reports, including incident reports, in compliance with agency procedures.
· Oversees maintenance of apartments, inventories furniture, and alerts program director of maintenance problems.
· Attends in-service training as requested by supervisory personnel.
· Provides coverage as needed for other staff on the program team.
· Serves as “Qualified Mental Health Staff Person”
· Duties as assigned by supervisor.

Bilingual Case Manager - Washington Heights CORNER Project

POSITION: BILINGUAL CASE MANAGER
Salary: $42,000– plus benefits

Position Description:
• Provide active substance users with short and long-term case management and supportive counseling using low-threshold, nonjudgmental, and culturally relevant methodologies
• Conduct participant intakes and assessments
• Make outside referrals and engage in follow-up activities for each encounter, provide escorts and advocacy when appropriate
• Conduct individual and group sessions with special focuses on wellness, mental health, sexual health, HIV transmission prevention and care, Hepatitis transmission prevention and care, risk reduction, safer injection practices, etc.
• Maintain proper documentation including audit-worthy client files
• Network and establish treatment and referral resources within New York City
• Expand aspects of service provision to special populations (HIV/AIDS, drug users, homeless, LGBTQ, sex workers, etc.)
• Provide trainings as appropriate to local CBO’s and attend local meetings to present on program activities and/or to seek out current information about disease prevention, treatment and care
• Orient and train staff as needed or as requested
• Support volunteers and peers as needed or as requested
• Assist with proposal development and program evaluation
• As part of a team, share in organizational responsibilities including but not limited to serving as a member of the crisis intervention team, general administrative work, general maintenance/cleaning of our office location, and volunteer training and coordination.

Medicaid Service Coordinator - Job Path

Job Description:
The Medicaid Service Coordinator facilitates a person-centered approach for support, advocacy, and access to community, employment, medical and personal resources for individuals with a range of developmental disabilities by working with individuals and their families.

Responsibilities include:
• Developing, implementing, maintaining and reviewing the person’s ISP (Individualized Service Plan) every six months, or as needed.
• Coordinating appointments and meetings with service providers to develop the ISP and monitoring progression of person’s valued outcomes.
• Making referrals based on the person’s needs.
• Providing information on services and supports available to the individual and his/her family.
• Scheduling meetings with the individual at least three times a year including one home visit, or as needed.
• Arranging for clinical and other assessments and updating as needed.
• Accompanying the individual to new programs or services and assisting in the placement process.
• Documenting the monthly visits and other contacts with service providers.
• Maintaining and updating all files and records regularly
• Advocate for the rights of participants while ensuring all health, welfare and safety needs are met

Qualifications:
Educational: An associate’s degree in a health or human services field from an accredited college or university or a degree in nursing as a Registered Nurse or an associate‘s degree or a bachelor‘s degree who has a minimum of 60 credits toward a bachelor‘s degree in a field other than health or human services may meet this educational requirement if a minimum of 20 of his/her college credits are in health and human services.
Experiential: One year experience working with people with developmental disabilities or One year experience as a service coordinator/case manager with any population. The minimum experiential level does not have to be met if the person has a master‘s degree in a health or human services field.

Salary: Low to mid 30’s, depending upon degree and experience. Excellent benefits, including tuition reimbursement after nine months.
