Supported Housing Case Manager - The Bridge

Case Manager

Leading non-profit mental health, residential and rehabilitation agency in New York City seeks a Case Manager for its Supported Housing Program in the Bronx to provide rehabilitative services to a caseload of mentally ill adult residents with goal of assisting them to live independently; compliance with requirements, including facilitating appropriate admissions/discharges, maintaining census, documentation and incident reporting; conducting ongoing maintenance assessments of residents’ apartments; performing related administrative duties, including correspondence, reports, and record keeping; available for 24-hour crisis management and emergency beeper duty; adherence to all applicable law, rules, regulations, and approved Bridge policies and procedures. EOE/AA.


Requirements:

MSW or MS/MA in related field or BS/BA with 2 years mental health experience. Excellent oral and written communication skills. Bilingual (Spanish speaking) desirable.
