Part Time Social Worker - Hearthside Care Coordinators

Hearthside Care Coordinators, a geriatric case management practice is seeking a part-time social worker. Experience necessary in making home visits and coordinating services for older adults and people with disabilities residing in Manhattan and Brooklyn. Solid computer and organizational skills a must. Car required.
