Heights and Hill Community Council, a newly expanded community-based case management agency serving the frail elderly in Brooklyn is looking to hire an MSW Social Worker to carry a caseload primarily in Brownstone Brooklyn (Brooklyn Heights, Cobble Hill, Boerum Hill).

Social Worker
The Social Worker works under the supervision of the Assistant Director or the Team Leader/Supervisor and plays a key role in ensuring that agency programs are carried out effectively and efficiently. The Social Worker will work closely with other Heights and Hill staff concerning program effectiveness and together with the Executive Director, Assistant Director, Team Leader and other team members, plan and carry out agency activities and programs. The Case Manager will, with the approval of the Team Leader, attend community meetings, attend training sessions, participate in committees and work with professional staff of other agencies and organizations that provide services to the elderly.

Qualifications:

-Bachelor's degree (any field)
-Minimum nursing home experience
