• LMSW/LCSW and at minimum three years experience in a mental health clinic.

1. Performs case specific consultation.
2. Facilitates and provides office based and off-site trainings.
3. Assesses and identifies training needs, gaps in service linkage and community resources.
4. Participates in cross disciplinary cross systems trainings, discussions with ACS, contract agencies and community partners.

A. Conducts group sessions for students and parents on relevant and timely issues.
B. Meets with the Double Discovery program staff (i.e., counselors for Upward Bound and Talent Search) who present potential cases and receive instruction on how to deal with common issues that arise with students and parents. If more serious issues for a student arise, makes referrals to outside organizations and practitioners best able to serve the needs of the student.
C. Performs other related duties as assigned.

• Responsible for establishing a youth-friendly service environment for Member Services and Counseling which involves managing intake and assessment; transitioning new members into counseling and other designated program areas to meet their individual needs
• Maintain an agency-wide youth development focused framework for the provision of counseling services, including methods, modalities and strategies
• Oversee short and long-term services to address needs of young people effectively
• Implement quality assurance activities to capture documentation compliance, engagement of new members and satisfaction of young people with services
• Provide supervision to Member Services Coordinator
• Provide supervision to Senior Mental Health Counselors
• Provide clinical support to other counseling staff across the agency
• Provide training to Member Services’ interns
• Ensure compliance with funding sources and contracts, including timely submission of reports and data collection
• Other responsibilities as assigned by supervisor

• Providing psycho-educational groups for members of the various church communities
• Education of clergy on the identification, engagement and treatment of mental health disorders
• Brief counseling and engagement of individuals in need of mental health treatment
• Participation in focus groups on improving collaboration between clergy and mental health practitioners
• Participation in research activities related to this program

• Participates with interdisciplinary team members in developing and implementing program policies and procedures and preventing crises.
• Assesses the social and emotional needs of patients.
• Provides direct counseling/social services to students referred by self, health team, faculty, administration and other school ancillary staff.
• Tracks and follows up on all referrals made in and out of school.
• Coordinates social work services and patient referrals between the program and other agencies providing social and educational services in both the school and local community.
• Establishes referral network/linkages with outside agencies for both students and parents.
• Participates and occasionally presents at teacher meetings, assemblies, etc.
• Serves as an advocate for patients' rights within the clinic and serves as a mediator between parents, child and school.
• Remains available for crisis intervention social work as the need arises or arranges for such coverage by emergency social services.
• Records professional assessments, identifies problems, treatment plans and progress notes as required by NYSOMH regulations, policies and procedures, and the Children's Aid Society.
• Attends all weekly clinical meetings (e.g. Utilization Reviews, Disposition, Administrative).
• Ensures that all patients, visitors, guests, as well as co-workers, receive personalized, prompt attention and are treated with receptiveness, dignity and respect, no matter how limited the contact.
• Will operate the on-call emergency phone on a rotating basis.
• Implements philosophy and objectives of program.
• Performs other related duties, as required.

• Triage all referrals for clinical acuity, gather demographic and insurance information and maintain appropriate computer records.
• Maintain list of vacancies and available hours for accurate timely screening visit assignments.
• Track admissions and discharges to programs and update caseload lists as changes occur.
• Maintain waiting lists, lists of disposed cases, data logs and distribute monthly reports.
• Respond to inquiries from referral sources about status of waiting list and screening appointments.
• Attend weekly and monthly intake meetings.
• Conduct utilization reviews.
