Duties:
Critical Time Intervention (CTI) is a new clinical program being developed for frequent users of the psychiatric emergency department at the Columbia University Medical Center. Individuals enrolled in CTI will receive recovery-oriented case management services for up to 6 months following discharge from the ER in an effort to try to decrease utilization of the ER and increase engagement in outpatient treatment among frequent users of psychiatric emergency services. The CTI specialist will be responsible for providing recovery-oriented case management services to clients, many of whom will have co-occurring disorders (mental illness and substance use disorders) and some of whom will be homeless. Responsibilities will include: engaging clients in a collaborative relationship, conducting assessments, developing treatment plans, assisting clients in applying for housing and other entitlements, accompanying clients to appointments with service providers, advocating for clients, helping clients further develop their support networks, and assisting clients in developing crisis management and other life skills. The Double Discovery Center provides academic programs for New York City youth in grades 7-12, with a focus on a population that is low-income, first generation college-bound. The part-time social worker provides counseling services to Double Discovery students, students' families, and graduates of the program. The incumbent is available on-site for scheduled or drop-in individual counseling sessions. In addition, the social worker conducts group sessions for parents and students on relevant and timely issues. The social worker conducts meetings with the Double Discovery staff.
DUTIES AND RESPONSIBILITIES

The Yale Child Study Center is pleased to offer a one-year Post-MSW Graduate Training Fellowship to begin July 1, 2009. The mission of this training program is to advance practice competency, professionalism, and leadership in the field of clinical social work. The Child Study Center is a multi-disciplinary department in the Yale School of Medicine that provides training, clinical services for children and families, and conducts research in child psychiatry and related disciplines. As a not-for-profit child welfare agency, the mission of Harlem Dowling- West Side Center for Children and Family Services is to serve and assist children and their families in crisis and distress.
Our services include foster care, adoption, preventive services and related assistance to children and their families to enable them to live in a stable, nurturing environment.
Presently I am looking to hire a Social Worker in my program. TFC is an intensive case management program for children who have an Axis I diagnosis. My program has a capacity for 40 children and 5 teams of workers serve eight children each. Each team consists of a Social Worker (a Master's level worker) and a Sociotherapist (a Bachelor's level worker). The teams work to coordinate services for each child surrounding the child's needs. These needs include the following areas: mental health, education/employment, recreation, medical, etc.
The Social Workers devise a treatment plan/ABC plan for each child and treat the targeted behaviors and issues, providing counseling to the child/youth, foster family and birth family. The Social Worker works in collaboration with community systems to ensure the well being and safety of the child, as well as working towards permanency for the child. Visits occur in the child/youth's home and school and in the community and office.
I am looking for an individual who is dedicated to the welfare of children and who exhibits motivation, commitment and caring. This job requires sound writing and communication skills. Bilingual staff is desirable to cater to our Spanish speaking parents and youth.
