ABOUT BRC OUTREACH:
BRC Homeless Outreach works around the clock, motivating homeless individuals sleeping in public spaces to take a step toward reclaiming their life. Outreach offers a hand-up, not hand-out, and every 5 hours of every day a person who once lived in a public space accepts our outstretched hand and takes a step inside. BRC operates several residences for individuals living with mental illness. Fulton House is one of three BRC residential programs with a combined capacity of 104 which are licensed by the New York State Office of Mental Health as Community Residences. While residing in these programs, individuals gain insight into their illness, and develop the skills to manage their health and live independently. Many resume their educations, and obtain employment. As their health and confidence improves, they graduate to their own apartment or to a shared-living program.
DUTIES/RESPONSIBILITIES:
Plan and participate in recreational activities in house and within the community; escort residents to various entitlements appointments; facilitate workshops related to life skills; assist with the scheduling of events and weekly social planning of groups; attend various interdisciplinary meetings in house and in the community (hospitals, treatment facilities, day treatment, etc.). The Social Worker works under the supervision of the Assistant Director or the Team Leader/Supervisor and plays a key role in ensuring that agency programs are carried out effectively and efficiently. The Social Worker will work closely with other Heights and Hill staff concerning program effectiveness and together with the Executive Director, Assistant Director, Team Leader and other team members, plan and carry out agency activities and programs. The Case Manager will, with the approval of the Team Leader, attend community meetings, attend training sessions, participate in committees and work with professional staff of other agencies and organizations that provide services to the elderly.
